I have just received a reminder from the GP regarding my Chronic Health Plan. The plan may be helpful to others in this discussion forum as it appears that many others also have multiple health issues that would make them eligible for the government subsidies. I have attached below a copy of the email with some of the details of the plan. To access the benefits have a chat to your GP.
